DVD COMING SOON!

Oregon Health Decisions is excited to announce that we are the proud recipient of a Spirit Mountain Community Fund grant. The grant will enable us to produce an Advance Directive DVD to help individuals better understand their choices for life-sustaining treatment and talk about them with their loved ones and health care provider. Spirit Mountain Community Fund is the philanthropic arm of the Confederated Tribes of Grand Ronde.
