Advance Directive Your Life. Your Decisions. ™ with the KEYConversations™ Planning Guide is divided into three “easy to use” steps:

The KEYConversations™ Planning Guide helps you express how you want to be treated if you are seriously ill and unable to speak for yourself. Many people shy away from preparing an Advance Directive, perhaps because it is difficult to think about death, or they aren’t sure what their end-of-life wishes are, or don’t know how to go about doing it. Taking time to think and talk about what medical treatments you would or wouldn’t want can be a blessing for your loved ones.

Having an Advance Directive gives you a voice in decisions about your medical care. Yet only a quarter or less of Americans have filled out Advance Directives. Without these documents, choices may be left up to a doctor or someone appointed by a judge, a person who might not know your values, beliefs, or preferences. Not only is it possible you receive care that does not reflect your wishes, but it may be a great burden on a loved one who is forced to make difficult decisions on your behalf without knowing what you would want. Completing your Advance Directive will give your loved ones an important gift – peace of mind.

The KEYConversations™ Planning Guide walks you through the process, explains the medical terms and procedures you will need to know, helps you determine what kind of end-of-life care you would want and even provides Oregon’s legal forms to complete your Advance Directive.
